Abstract Respiratory dysfunction is an underrecognized component of Parkinson’s disease (PD), but its relationship with neurological disease severity, motor phenotype, and systemic inflammation remains incompletely understood. This study aimed to characterize pulmonary function in patients with PD and identify clinical and inflammatory factors independently associated with impaired spirometric performance. Methods This single-center observational study included 93 patients with PD. Demographic and clinical characteristics, Hoehn–Yahr stage, Unified Parkinson’s Disease Rating Scale (UPDRS) score, disease duration, spirometric parameters, and laboratory markers, including C-reactive protein (CRP), albumin, and haemoglobin, were recorded. Respiratory and disease-specific outcomes were assessed using the St George’s Respiratory Questionnaire, COPD Assessment Test, modified Medical Research Council dyspnoea scale, and Parkinson’s Disease Questionnaire-39. Patients were categorized according to Hoehn–Yahr stage, motor phenotype, and pulmonary function status, with reduced pulmonary function defined as FEV₁
